Wire rope barrier systems (also called cable barrier systems) can be struck from either side by an errant vehicle and they will function as intended. This, and their narrow width, makes them a useful system for medians (wide or narrow).  

There are also a variety of back-to-back flexible w-beam systems available that can be struck from both sides, and of course concrete systems that are designed for impact from both sides.  

However, the selection of the appropriate median barrier needs to take into account a raft of factors, including; offsets to opposing lanes, horizontal and vertical alignment, operating speeds, length between access points, whole-of-life cost, consistency within a route, traffic volume and likely nuisance impacts, ability to maintain the barrier, and the road and percentage of motorcyclists and heavy vehicles… just to name a few.
