A few years ago the Safe System Solutions Pty Ltd team were undertaking testing at Crashlab in NSW. Before we were allowed anywhere near the site, there were extensive risk assessments, Safe Work Method Statements, pre-start briefings, equipment checks and contingency planning. My testing colleague, Tana Tan (PhD), spent weeks working through the documentation and coordinating the safety processes to make sure everyone involved went home safely.
That’s why this video makes me so confused.
I’m not going to speculate on what happened because a short video never tells the full story. But it does raise an important question: what failed in the planning or control measures that allowed people to be exposed to this level of risk?
(The trusty internet tells me that no one was killed or seriously injured, which I hope is true but honestly amazes me)
