Kenngineering Notes: I’m going to need some help understanding this one

We get sent a lot of videos to turn into Safe System Snippets (we’re up to 548 so far – you can check them all out here). But this one, sent through last week, has genuinely stumped us.
It’s just hard to tell what’s actually happened here. Is it a surface issue? Is it an intentional act? Or something else entirely?\
